The vital records are obtained from civil registration services in the municipalities where the acts have been prepared. For example, the mayor of the place of birth registration for a birth certificate, the City Hall celebration of marriage for a marriage certificate, etc..
records of vital statistics (birth records, marriage and death) are for the departments and communities overseas, prepared in triplicate: the first is owned by municipalities, the second by the court high instance (or the court of first instance) locally relevant and the third by the Ministry of Overseas.

name, surname, date, place of birth and full names of parents (maiden name for mother) of the person concerned. Applications must be accompanied by a stamped envelope with your name and address.
